JOB SUMMARY:  Social Services provides effective clinical services to include group, individual and family therapy as ordered by the Physician. Directs the overall social and activity therapies of the hospital. 

ESSENTIAL JOB DUTIES:  To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation.  

• Work cooperatively with discharge planners and other community referral sources to assure provision of appropriate social services to patients admitted to the hospital

• Interview patient, families or significant others to complete a thorough psychosocial assessment and collect data pertinent to treatment and discharge planning within 72 hours of admission

• Provide individual, group and family therapy to address specific issues identified in the treatment plan

• Complete necessary documentation corrections as identified by the Compliance Officer within policy guidelines

• Audit active and closed charts for compliance

• Actively participate in multidisciplinary treatment team meetings weekly

• Prepare MTP review summaries weekly to review patient progress and establish the medical necessity of continued treatment

• Maintain knowledge of appropriate discharge and aftercare resources

• Conduct monthly in-service with the Activity Therapist and Director of Therapy

JOB REQUIREMENTS:

• Maintain accurate and up to date knowledge of all relevant regulations and standards regarding the provisions of clinical services. 

• Work cooperatively with the Administrator and Physician to ensure all relevant regulations and standards are met, including Medicare, state licensure requirements and Unity Psychiatric Care’s policy and procedure.

• Adheres and respects all policies and procedures regarding anonymity and confidentiality of all patient records past and present. This covers any written or verbal communications regarding patients identify, address, and situation.

• Functions in a friendly, supportive, courteous, respectful, cooperative, and professional manner with patients, families, physicians, referral sources, visitors, coworkers, and management.

• Promote, adhere, and monitor for a drug and violence free workplace.

• Adhere to ethical and legal considerations of discipline practiced. 

• Develop and implement individualized treatment plans utilizing therapeutically recognized interventions. 

• Operate as a member of a multidisciplinary treatment team and patient advocate. 

• Communicate professionally and effectively with other health care professionals, aftercare resources, and family members.

 

Required Skills:

• Comprehensive knowledge of patient rights, characteristics of persons served and interventions

• Comprehensive knowledge in completing and conducting psychosocial assessments, social history, and assessing high social risk cases

• Comprehensive knowledge in counseling for patient, family, and caregivers regarding mental health

• Comprehensive knowledge of treatment plans, goals, and modalities that are patient specific

• Comprehensive knowledge and sensitivity in patient confidentiality, and cultural diversity

• Knowledge of mental health treatment issues, including DSM-V diagnostic criteria and psychotropic medications

• Comprehensive knowledge in hospital procedures, federal and state laws, accreditation requirements or psychiatric hospitals

• Excellent verbal and written communication skills

• Knowledgeable of legal aspects of clinical practice and professional standards

• Knowledgeable and demonstrates the mission of the hospital

• Promotion of the reduction of stigma of mental illness

• Knowledgeable of the persons served and their rights

• Advocate for the needs of the persons served

 

 

Required Computer Software/Equipment used:

• Advanced Microsoft Suite, including Excel, Access, Power Point and Word

• Outlook

• Standard office equipment

 

REQUIRED QUALIFICATIONS: 

Education/License:  

• Master’s degree in social work from an accredited school of Social Work required

• LCSW or another clinical license or certification preferred

• CPR and CPI certification required (or training completed prior to direct patient care) with yearly refreshers 

• Restraint and Seclusion training (or training completed prior to direct patient care)

Experience:   

• Two (2) or more years of clinical experience with severe and persistent mental illness and/or geriatrics preferred

• Supervisory experience preferred

PHYSICAL REQUIREMENTS:

Ability to Perform Essential Functions of the Job

• Ability to lift to 40 pounds

• Subject to standing, walking, sitting, bending, reaching, kneeling, pushing and pulling
