Social Services Director

Skilled Nursing / Long-Term Care | Colorado

A well-established skilled nursing and rehabilitation facility in Colorado is seeking a compassionate and experienced Social Services Director to lead psychosocial services and support resident-centered care in a long-term care setting.

This leadership role is ideal for a professional who is passionate about resident advocacy, care coordination, and supporting both residents and families throughout their stay.


Key Responsibilities

  • Oversee and direct all social services operations within the facility
  • Conduct psychosocial assessments, care planning, and required documentation, including MDS input
  • Advocate for residents’ emotional, social, and mental well-being
  • Collaborate with the interdisciplinary team to support care conferences and care transitions
  • Assist residents and families with discharge planning, transfers, and community resources
  • Maintain compliance with state and federal regulations related to social services
  • Build strong relationships with residents, families, and community partners
  • Support resident engagement initiatives and quality-of-life programming

Qualifications

  • Bachelor’s degree in Social Work or related field preferred 
  • Current or eligible licensure in the state of Colorado (LSW/LCSW preferred)
  • Prior experience in skilled nursing or long-term care strongly preferred
  • Working knowledge of MDS, care planning, and regulatory requirements
  • Strong communication, organizational, and leadership skills
