Physical Therapist Assistant (PTA)

Wesley Gardens Nursing Home is looking for a PER DIEM Physical Therapist Assistant (PTA) to join our seasoned professional therapists in our in house rehab department

Located at 3 Upton Park, Rochester, NY 14607

Job Responsibilities:

1. Provides skilled therapeutic intervention and services under the supervision of a Physical Therapist.
2. Follows all regulations and guidelines of facility including compliance with confidentiality and HIPPA.
3. Adheres to discussed scheduled hours and conducts self in a professional manner at all times.
4. Provides documentation of patient’s condition, and provide caregiver/patient education as appropriate.
5. Regularly discusses with supervising Physical Therapist patient’s progress and any concerns or guidance needed.
6. Ensures safe treatment practices and monitors patient’s engagement and responses to treatment, modifies as necessary.

Requirements include:

1. Valid NY professional license
2. Graduated from an accredited Physical Therapy Assistant program
3. Preferred experience with rehab optima and skilled nursing facilities
4. New graduates are welcome
