MDS Nurse RN/LPN
Job Summary:
Join our 5-star skilled nursing facility, Winchester Rehabilitation and Nursing Center, as an MDS Nurse RN/LPN! Responsibilities:
Conduct and coordinate the development and completion of the resident assessment
Conduct resident assessments in accordance with current rules, regulations, and guidelines
Implement resident CAA's
Participate in facility survey readiness and throughout the survey
Work with our interdisciplinary care team to develop a comprehensive assessment and care plan for each resident
Requirements:
Current unencumbered State of Massachusetts RN or LPN License
Two years of MDS and PPS experience
Care Planning experience
