Social Services Director
Rose City Nursing and Rehab | Lancaster, PA

Rose City Nursing and Rehab is seeking a dedicated Social Services Director to join our nursing home in Lancaster, PA.

The Social Services Director serves as a liaison and advocate for residents’ social interests and is responsible for planning, organizing, implementing, evaluating, and directing the overall operations of the Social Services Department.

Position Details

  • Full-Time

Job Functions

  • Develop comprehensive social assessments and care plans identifying residents’ medically related social and emotional needs with realistic goals and interventions

  • Actively participate in pre-admission and admission processes

  • Provide compassionate support to residents and families during adjustment, crisis, or special circumstances

  • Maintain accurate documentation of residents’ behavioral and psychosocial status

  • Provide individual and group support services for residents and families

  • Establish and grow a volunteer program to meet social service needs

  • Orient residents and families to the facility and assist with transition and adjustment

Qualifications

  • Bachelor’s Degree from an accredited school in Social Work or a related field required

  • Strong knowledge of social service practices, procedures, laws, and regulations in long-term care

  • Ability to work independently with strong interpersonal and communication skills
