Presbyterian Homes & Services - Optage Hospice is seeking a Registered Nurse to join our team as a Hospice Visit Nurse.

We are seeking experienced hospice RN's who wish to be a part of a care team that focuses on supporting individuals and their families at end of life with compassion and dignity, that can adapt to priorities and work routines while supporting personal preferences of clients. The Hospice Visit Nurse position is a supportive role to the Hospice Interdisciplinary Group or IDG team. The ideal candidate will have knowledge of hospice philosophy, hospice Medicare benefit, the dying process, and experience in hospice case management. Flexibility is a top priority of the Hospice Visit Nurse as this position provides opportunity for highly diverse nursing visits based on client need, but can also serve as an admission nurse or provide coverage for RN Case Manager, in addition to end of life visit needs. Will cover visits throughout the metro area, so must be comfortable with driving. This position requires highly developed communication skills that includes the patient, family members of patient, and members of IDG team, On-Call nurses, plus the supportive staff at the various housing choices from which patients live.

This full-time benefit position (.8 - 1.0) includes working some weekends with regularly scheduled days off and an occasional weekday on-call rotation.

Responsibilities:
The Hospice Registered Nurse (RN) ensures quality and safe delivery of hospice services to our clients in their home, skilled nursing facility or assisted living setting and is responsible for completion of assigned visits and admissions based on staffing and patient/family needs.

The Hospice Registered Nurse (RN) ensures the client's individualized plan of care is implemented and makes necessary revisions to address all problems. They provide coordination of hospice services, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs. Hospice RNs utilize assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life for the patient/family.

Typical duties include providing ongoing assessments of physical, psychological, social, and spiritual needs of the patient and family, provide education, supervision and counseling to the client and their family/caregiver regarding care needs as appropriate to client's needs, providing accurate and appropriate documentation of patient/family services.

Qualifications:
  1. Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices. One year clinical nursing experience preferred. One to Two (1-2) years of experience in Hospice or Home care a plus.
  2. Ability to work with patients and their caregivers during end of life while in the home, hospital and other facility settings. Shows sensitivity to persons in crisis while demonstrating good listening skills
  3. Ability to work independently with self-discipline as well as cooperatively as part of an interdisciplinary team.
  4. Excellent interpersonal, communication, and organizational skills including the ability to handle multiple priorities accurately and timely to facilitate client and program needs.
  5. Demonstrated knowledge of home care or hospice and palliative care clinical practices. Strong knowledge of pain & symptom management preferred.
  6. Demonstrated Computer skills: able to document effectively in EM.
  7. Must have valid driver's license in state of residence and proof of auto insurance.
  8. Demonstrated compatibility with PHS's mission and operating philosophies.
  9. Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
