Presbyterian Homes & Services - Optage Hospice is seeking a LPN to join our team as a Hospice Visit Nurse.

We are seeking LPNs who wish to be a part of a care team that focuses on supporting individuals and their families at the end of life with compassion and dignity, and that can adapt to priorities and work routines while supporting personal preferences of clients. The Visit Nurse position provides the opportunity for diverse nursing visits based on client need or end of life visit needs to our patients throughout the metro area.

This full-time benefit position (.8 - 1.0) includes working some weekends with regularly scheduled days off.

As an LPN, you will play a critical role in caring for hospice clients- not only physically, but emotionally, socially, mentally, and spiritually. Responsibilities:
The Licensed Practical Nurse (LPN) is responsible for the provision and monitoring of high quality hospice care and services to clients consistent with regulations and established clinical best practices.

The Hospice LPN ensures quality and safe delivery of hospice services to our clients in their home, skilled nursing facility or assisted living setting and is responsible for completion of assigned visits and admissions based on staffing and patient/family needs..

Typical duties include providing delegated nursing visits, provide education & support to the client and their family/caregiver regarding care needs as appropriate to client's needs, & providing accurate and appropriate documentation of patient/family services.
Qualifications:
  • Licensed Practical Nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
  • Current CPR certification.
  • Interest in gerontological nursing, rehabilitative and restorative clinical practices.
  • Must have valid driver's license in state of residence and proof of auto insurance.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
