PlanMerica Home Care Solutions, a nonprofit organization dedicated to providing compassionate and high-quality home care and home health services, is seeking a Part-Time Patient Advocate Coordinator (PAC) to join our team. As a PAC, you will play a vital role in promoting independence, dignity, and well-being for our clients, including seniors, individuals with disabilities, and those requiring medical or non-medical assistance at home.

Key Responsibilities:

  • Provide transportation to school and therapy for clients Monday through Friday, ensuring timely and safe arrival

  • Coordinate scheduling and communicate with clients, healthcare providers, and caregivers to ensure seamless care delivery

  • Advocate for clients' needs and preferences, promoting autonomy and dignity in their daily lives

  • Foster strong relationships with clients, families, and caregivers to ensure exceptional customer service

  • Participate in community outreach and education initiatives to promote PlanMerica's mission and services

Requirements:

  • 0 years of experience in patient advocacy, healthcare, or a related field

  • High school diploma or equivalent required
