Position: Social Worker
Location: Long Beach, New York
Shift: Full-Time | Tuesday–Saturday | 9:00 AM – 5:00 PM
Salary: $68,000 per year

Position Summary:

We are seeking a compassionate and dedicated Social Worker to join our interdisciplinary care team. This role plays a critical part in supporting residents and families as they navigate the challenges of illness, placement, and transition. The ideal candidate will be a proactive problem solver with experience in a skilled nursing facility, strong discharge planning expertise, and a solid background in healthcare or medical settings.

Key Responsibilities:

  • Conduct psychosocial assessments for all newly admitted residents and families to identify treatment needs.

  • Provide social work services to residents/families experiencing adjustment issues or complex emotional responses to illness or placement.

  • Develop and implement individualized care plans in collaboration with residents, families, and the care team.

  • Represent psychosocial issues and intervention strategies at interdisciplinary care plan meetings.

  • Build strong collaborative relationships with nursing, therapy, and administrative staff to improve resident outcomes.

  • Facilitate therapeutic family meetings and provide ongoing communication to address concerns and support engagement.

  • Ensure compliance with all federal, state, and facility policies, documentation standards, and best practices.

  • Participate in staff orientation, education sessions, and in-services to promote team understanding of psychosocial care.

  • Serve as a liaison for residents and families, advocating for their needs and assisting in conflict resolution.

  • Assist in discharge planning by connecting residents/families with appropriate community services and support systems.

Qualifications:

  • Master’s degree in Social Work (MSW) preferred.

  • Minimum of 1 year of experience in a skilled nursing facility required.

  • Discharge planning experience required.

  • Experience in a healthcare or medical setting required.

  • Excellent interpersonal, communication, and organizational skills.

  • Must be physically present in the facility to perform duties.
