Position: Social Worker
Location: Long Beach, New York
Shift: Full-Time | Tuesday–Saturday | 9:00 AM – 5:00 PM
Salary: $68,000 per year
Position Summary:
We are seeking a compassionate and dedicated Social Worker to join our interdisciplinary care team. This role plays a critical part in supporting residents and families as they navigate the challenges of illness, placement, and transition. The ideal candidate will be a proactive problem solver with experience in a skilled nursing facility, strong discharge planning expertise, and a solid background in healthcare or medical settings.
Key Responsibilities:

Conduct psychosocial assessments for all newly admitted residents and families to identify treatment needs.

Provide social work services to residents/families experiencing adjustment issues or complex emotional responses to illness or placement.

Develop and implement individualized care plans in collaboration with residents, families, and the care team.

Represent psychosocial issues and intervention strategies at interdisciplinary care plan meetings.

Build strong collaborative relationships with nursing, therapy, and administrative staff to improve resident outcomes.

Facilitate therapeutic family meetings and provide ongoing communication to address concerns and support engagement.

Ensure compliance with all federal, state, and facility policies, documentation standards, and best practices.

Participate in staff orientation, education sessions, and in-services to promote team understanding of psychosocial care.

Serve as a liaison for residents and families, advocating for their needs and assisting in conflict resolution.

Assist in discharge planning by connecting residents/families with appropriate community services and support systems.
Qualifications:

Master’s degree in Social Work (MSW) preferred.

Minimum of 1 year of experience in a skilled nursing facility required.

Discharge planning experience required.

Experience in a healthcare or medical setting required.

Excellent interpersonal, communication, and organizational skills.

Must be physically present in the facility to perform duties.
