Job Summary and Qualifications
- Helps to develop and monitor the implementation of the patient safety-risk management program. Identifies risk exposure by assuring the planning, development, and support of an effective event reporting system and establishing effective communication with surveillance departments e.g. Infection Control, Employee Health, Quality Management, Safety, and Medical Records.
- Assists with the management of the hospital event reporting system providing support to select committees, units or departments in order to mitigate risks and advance the patient safety agenda across the hospital.
- Collects, tracks, trends, and performs data analysis utilizing multiple data systems and tools to create actionable reports and improvement plans.
- Assist in identifying, trending, and investigating patient safety incidents, medical errors and potential risks in the facility;
- Participates in and coordinates event investigation and analysis and proactive risk assessments e.g. Failure Modes Effects Analysis.
- Conducts chart reviews for the relevant discipline/program to investigate patient safety and adverse events.
- Leads activities to improve event reporting and response to near miss events.
- Assist with administering the Patient Safety Program on a day to day basis to include review and follow up with RM Notification reports for timeliness and completeness.
- Conducts and participates in Patient Safety/Risk Management educational programs, web-based and in-person, that provide front line staff with the tools and equipment to be accountable for their role in patient safety and risk activities.
- Clearly articulates ideas and patient safety concepts in non-judgmental and non-confrontational ways.
- Display integrity to maintain effectiveness and build the trust that is essential to the honesty and transparency of the Patient Safety Program.
- Maintains a current, up-to-date understanding of patient safety theory, methods, standards and regulatory requirements, which is essential along with a commitment to lifelong learning to maintain expertise in a rapidly changing field.
- Completes the work of patient safety/risk management with the objective in mind to advance patient safety and control and minimize risks at the facility.
- Assures timely reporting of Patient Safety Work Product (PSWP) to the Patient Safety Organization as assigned.
- Assists with regulatory and accreditation/certification visits by retrieving clinical data from the patient medical records, escorting surveyor(s) to the departments, setting up requested interviews with staff, and leadership notifications e.g. division and corporate of the visit.
- Acts as liaison with internal departments and external accrediting bodies, health and safety agencies, insurance companies, and legal counsel.
What qualifications you will need:
- B.A. or B.S. required
- Clinical background preferred
- Preferred current CPHRM or CPPS certification. If not already CPPS, must obtain certification within 12 months of hire. Maintain CPPS while in role.
- Minimum 2 – 3 years’ experience in a healthcare setting required. Healthcare patient safety, risk management, or quality improvement preferred.
