Under the direction of the Clinical Manager :

• This position assumes the lead role in admitting, discharging, and transferring of patients, collaborates with the Physician, Case Manager and inter-disciplinary health care team members involved with discharging the patient.
• Responsible for providing discharge instructions ,reviewing medications and follow –up appointment with patients/families that are being discharged from the hospital. This position will assume bedside RN responsibilities as needed.
• Anticipates the needs of the staff members and proactively offers assistance. Acts as a role model to team members through effective communication skills and assisting in their development of such skills. Collaborates with all members of the patient care team by actively communicating and reporting pertinent patient care information and data in a comprehensive manner.This position is responsible for the improvement of the admission, discharge and transfer process to enhance patient flow within the facility.
