Per Diem Rates:
The telehealth visits for community = $150 (which is an average compensation).
In person visits are compensated = $150- $210 (with $180 being an average FFS)
Orientation hourly rate: $90/Hour
This field position will cover Brooklyn and Queens regions.

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.
- Provide and/or coordinate primary clinical care services for assigned high riskmembers and facilitates supportive care during transitions from acute or sub-acute care setting
- Acts as a team player, member’s advocate, and a clinical lead while working closely withInterdisciplinary Team to coordinate member’s care and optimize health outcomes
- Acts as resource and contact for member/designated representative regarding all aspects of memberindividualized care plan and disease process
- Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
- One to two years clinical experience in role of nurse practitioner required
- Minimum one year in geriatric specialty
- Previous clinical RN experience in acute, subacute and/or long term care setting preferred
- Prior experience in manage care a plus
- NYS NP License, Registration, DEA certificates
- Driver license and car required
