Summary:

Directs the department's activities and resources to ensure alignment with the mission, values, and objectives of the department and organization. Plays a pivotal role in aligning patient psychosocial care coordination, discharge planning, organizational goals, regulatory compliance, and financial stewardship. This role promotes a holistic approach to patient and family support and requires strong leadership skills to inspire and guide the social work and case management teams in achieving shared goals while fostering collaboration.The Director oversees social work and case management operations, executes initiatives to support safe and timely transitions of care, reduce avoidable readmissions, and ensure appropriate resource utilization. Manages a team working under conditions requiring quick, informed decision making.

Responsibilities:

Prepares and evaluates departmental budgets and ensures the department operates in compliance with allocated funding. Coordinates and directs internal/external audits.Plans and monitors staffing activities, including hiring, orienting, evaluating, disciplinary actions, and continuing education initiatives.Manages and evaluates department operations, including information technologies, service determination, and complaint management to achieve performance, quality, and patient centered care objectives.Develops and implements departmental goals, plans, and standards consistent with clinical, administrative, legal, and ethical objectives of the organization.Leads individuals and work groups to continuously improve performance by creating an environment that ensures application of quality principles and techniques to achieve desired outcomes.Monitors performance feedback and plans, develops and implements activities to improve the individual and collective performance of the staff in providing services to patients and families. Directs and develops staff to look at service delivery workflows and processes and identifies opportunities for improvement of the outcome metrics.

Complexity of Work: Requires critical thinking, effective communication skills, decisive judgment, and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.Requires expert level understanding of social work practice, case management principles, behavioral health and psychosocial assessment, crisis intervention, and discharge planning. Ability to balance patient needs with system priorities while maintaining compliance with regulatory requirements. Knowledgeable of state and federal regulations governing discharge planning and social work practice, hospital information systems, staff development techniques, and performance improvement processes.Required Work Experience: Five (5) years of experience in a social work or case management setting with, at least, two (2) years of leadership experience within social work, care coordination, or related operational oversight.Other Information: Additional Licensure Info: Licensed Clinical Social Worker (LCSW) – State of Florida (or equivalent state issued Social Work licensure such as LMSW, LISW, or LSW per state requirements).
