Independently assesses, plans, implements, and evaluates the nursing care of patients from admission, transfer or discharge. In collaboration with the patient and family, provides theory-based professional nursing care and coordinates are delivery with the physician and other members of the health care team. Serves as a resource professional to all staff assigned to unit. Follows department specific policies and procedures, and maintains competencies of assigned department.

Minimum Qualifications

Graduate of an accredited nursing program.

Current license as a Registered Nurse in the state of Maryland or a Multistate Compact state or be license eligible.

BLS certification required by completion of introductory period.

One year of experience in a related clinical area

Primary Duties and Responsibilities
-
Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Ensures compliance with hospital/facility policies and procedures and governmental/accreditation regulations.

Assures accurate communication accurately in verbal and written format. Clearly and accurately documents pertinent information in the patient's medical record. Conducts chart reviews and takes appropriate action when discrepancies are found.

Contributes to optimal patient outcomes as coordinator of a unit-based health care team. Supervises the provision of nursing care to assigned patients. Consistently demonstrates effective leadership behavior.

Maintains patient privacy and confidentiality. Facilitates collaboration among other health team members fostering effective communication with other disciplines.

Coordinates appropriate care and placement of patients from admission to discharge. Provides for the safe and efficient transport of patients to other departments. Facilitates the care of high-risk patients. Arranges for the transfer of patients interdepartmentally as well as to other facilities. Takes the lead in facilitating timely discharge.

Ensures that the care given by all staff members is consistent with unit standards. Supervises the performance of the delegated nursing task by licensed and unlicensed staff. Assists staff in delegation to time management skills. Identifies was to facilitate working relationships among all levels of staff.

Handles patient and family complaints real time, following up with nursing management. Conducts daily patient customer service rounds. Plans for and implements service recovery if necessary.

Provides for the emotional support of patients and families during the crisis of illness. Models patient/family advocacy.

Reviews and revises daily staff and assignments as needed based upon acuity and patient flow activity. Assigns break and meal periods.

Supervises and provides direct patient care in accordance with accepted nursing standards while utilizing the nursing process. Oversees and maintains awareness of serious patient issues. Identifies areas needed for improvement in patient care delivery and plans for effective change. Serve as a resource to staff, colleagues and other departments. Demonstrates timely problem solving and decision-making. Communicates relevant issues with nursing management.
