Job Summary

The home health aide is a paraprofessional member of the interdisciplinary team who works under the supervision of registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The home health aide is responsible for observing the patient, reporting these observations and documenting observations and care performed. The home health aide will be assigned in a manner which promotes quality, continuity and safety in patient care. Follows department specific policies and procedures, and maintains competencies of assigned department.

Primary Duties and Responsibilities
-
Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.

Assists with patient ADLs (i.e. bathing, dressing, skin care, positioning, feeding, ambulation, toileting and range of motion activities) according to the patient’s needs and the nursing care plan.

Provides support for and reinforces teaching to patient/caregivers

Ensures the safety of the patient at all times. Immediately reports any safety concerns.

Assists with light meal preparation, feeding and clean-up.

Reports any significant changes to the clinical office.

Documents the care provided and the patient’s response to care.

Respects patient/family/caregiver environment and patient's personal needs.

Participates in interdisciplinary team meetings, as requested by the Hospice Nurse Case Manager.

Performs other duties as assigned.
