Job Summary
  • Performs clinical support functions of pharmacy including clinical rounds, drug information, antibioticmonitoring, and chart review. Provides prescribed medications, drugs, and other pharmaceuticals andcompound medications in accordance with professional standards and practices. Performs the clinical andeducational functions of the Pharmacy Department.
Minimum Qualifications
  • Education/Training
    • Licensed as a pharmacist in the State of Maryland; advanced degree preferred.
  • Experience
    • 1 year clinical hospital pharmacy experience with understanding of pharmaceutical careconcepts.
  • License/Certification/Registration
    • Pharmacist license in the State of Maryland. Residency or certification preferred.
  • Knowledge, Skills & Abilities
    • Verbal and written communication skills. Basic computer skills preferred.
Primary Duties and Responsibilities
  • Administration: Ensures that medications are safely and accurately administered and makes recommendations for improvement to the Medication Safety Program. Advises professional staff to assure that the medication is being administered at the proper time, in the prescribed dose, and by the correct route. Advises the patient and clinical staff regarding any clinically significant, adverse reactions, or other concerns about administering medication. Discusses any unresolved, significant concerns about the medication with appropriate healthcare providers.
  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
  • INVESTIGATIONAL MEDICATIONS: Follows policies and procedures established for management of investigational medications, including storage, dispensing, and labeling. Participates in the evaluation of study protocols. Reviews and makes recommendations to assure compliance with the protocol, and participates in the education of staff regarding management of the medications. Reviews and assures understanding of investigational protocol prior to dispensing investigational medication.
  • Monitors the effects of medication(s) on patients to ensure that medication therapy is appropriate and minimizes the occurrence of adverse events and medication occurrences. Monitors and addresses each patient's response to his or her medication according to the clinical needs of the patient, and hospital policies and procedures. Gathers patient's own perceptions about side effects, and when appropriate, perceived efficacy.Responds appropriately to actual or potential adverse drug events and medication occurrences. Understands and takes appropriate action when an actual or potential adverse drug event and/or medication occurrence is identified Complies with internal and external reporting requirements for actual and potential adverse drug events and/or medication occurrences using the Medication Occurrence Reporting System, USP, FDA, ISMP, etc. Performs/conducts initial investigation upon receiving an adverse drug event and/or medication occurrence.
  • ORDERING: Ensures that only medications needed to treat the patient's condition(s) are ordered. Verifies that there is a documented diagnosis, condition, or indication for use for each medication ordered. Ensures that medication orders are complete, written clearly, and transcribed accurately. Adheres to and makes recommendations for written policies and procedures regarding the ordering and writing of medications. Verifies the essential elements of an order before processing Adheres to and makes recommendations for written policies and procedures regarding the use of verbal and telephone orders, use of unapproved abbreviations, and illegible orders.
  • Participates in multidisciplinary quality and service improvement teams.
  • Participates in the development of patient counseling, materials, and the development of hospital policy regarding patient consultation.
  • Participates in the evaluation of the hospital's medication management system for risk points and identifies areas to improve safety. Evaluates the literature for new technologies or successful practices that have been demonstrated to enhance safety in other organizations to determine if it can improve its own medication management system. Reviews and reviews internal reports to identify trends and opportunities to improve medication safety.
  • PATIENT INFORMATION: Obtains and integrates patient specific profiles with information required for safe medication management, in accordance with pharmaceutical care concepts. Gathers and documents information that should include: p patient's age, sex, height, and eight; correct medications, allergies, past sensitivities; diagnosis and co morbidities; relevant lab values; pregnancy and lactation status; and, other information as required by the hospital and/or for safe medication management. Participates in the exchange of information with appropriate health care professionals and staff Makes recommendations for ongoing change based on patient specific information.
  • Performs other duties as assigned.
  • Prior to dispensing, reviews all prescriptions or medication orders for appropriateness. This includes, but may not be limited to: appropriate drug, dose, frequency and route; duplicate therapies; potential for allergies or sensitivities; potential for drug, food or lab interactions; organizational criteria for use. Safely prepares medications utilizing proper techniques: employs appropriate precautions when preparing hazardous medications; employs techniques to assure accurate medication preparation; oversees non-licensed staff in the preparation of medications. Labels medications according to hospital policy, applicable laws and regulations, and standards of practice to ensure safe, accurate administration: labels medications for dispensing with at least the drug name, strength, amount, expiration date, patient name, patient location, and directions for use; dispenses medications in a safe manner; dispenses quantities that are minimally sufficient to treat patient's needs safely; dispenses medications in the most ready-to-administer form; adds medications to Pyxis as based on patient needs. Adheres to all laws, regulations, and hospital policies regarding the management of recalled, discontinued, unused, expired, or returned medications: removes discontinued medications from the unit; inspects returned medications for integrity before returning to stock; segregates outdated or recalled medications from pharmacy stock. Accounts for all unused, expired, or returned controlled medications according to the law.
  • SELECTION AND PROCUREMENT: Ensures that medications available for diagnosis or administration are selected, listed, and procured based on criteria. Evaluates/reevaluates medications using at least: indication for use, efficacy, safety, and cost. Educates healthcare providers and staff before a medication is made available for dispensing or administration within the facility. Maintains and/or makes recommendations for policies and procedures regarding the approval and procurement of non-formulary medications. Maintains and/or makes recommendations for policies and procedures that address medication shortages, outages, emergency and disaster situations. Maintains and/or makes recommendations for approved therapeutic substitution protocols.
  • STORAGE: Ensures that medications are properly and safely stored throughout the hospital. Abides by procedures assuring that unapproved and high risk medications, such as concentrated electrolytes and high alert drugs are not immediately available on a specific unit or care area, and specific precautions are taken to prevent inadvertent administration. Routinely inspects all medication storage areas according to hospital's policy to make sure medications are stored properly. Participates in medication recall procedures and education . Ensures that emergency medications and/or supplies, if any, are consistently available, controlled, and secure in the hospital's patient care areas. Adheres to the hospital policy that addresses the use of medications brought into the hospital by patients or their families.
