LaSante Health Center is seeking a Certified Nutritionist to join our team. The ideal candidate will provide personalized nutrition counseling to help patients achieve their health goals, manage chronic conditions, and promote overall wellness.
Responsibilities:
Assess and develop personalized nutrition plans for patients.
Educate patients on healthy eating, disease prevention, and weight management.
Support chronic disease management through nutrition.
Collaborate with healthcare providers for comprehensive care.
Lead nutrition workshops and stay updated on nutrition trends.
Maintain accurate patient records and provide food security resources.
Qualifications:
Certified Nutritionist or Dietitian
Prior healthcare or clinical experience.
Strong communication and interpersonal skills.
