Minimum Requirements:
- Associate's degree in physical therapy from an accredited educational program.
- Active Washington State PTA license required to start the position.
- BLS certification from AHA or ARC may be required.
- Home health experience preferred.
- Ability to complete and submit OASIS assessments and appropriately utilize ICD-10 codes.
- Experience working under the direction of a supervising Physical Therapist and collaborating with interdisciplinary home care teams.
- Availability for a Days schedule, Monday through Friday.

Responsibilities:
- Provide treatment per physician orders and plan of care under the supervision of the Director of Clinical Services.
- Work in conjunction with home care personnel in patient care planning.
- Direct and supervise Physical Therapy Assistants and Home Health Aides, as assigned.
- Take initial patient history and perform initial evaluations.
- Consult with physicians regarding changes in treatment.
- Instruct patients and family in home programs and activities of daily living.
- Participate in Quality Assessment and Performance Improvement activities as assigned.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition.
