Looking for a change of scenery and a chance to help others. Provide skilled physical therapy services in a home health setting in Pulaski, TN. Perform patient evaluations and deliver therapeutic interventions according to physician orders. Supervise and evaluate physical therapy assistants and home health aides as assigned. Maintain documentation of patient care and coordinate with interdisciplinary team members. Reliable transportation, valid driver’s license, and agency-required liability insurance may be required for home visits.
Minimum Requirements
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association or equivalent accrediting body.
- Current, active, and unencumbered Tennessee physical therapist license required to start the assignment.
- Current BLS/CPR certification from American Heart Association or American Red Cross.
- Experience providing physical therapy in home health or outpatient settings preferred. (2-3 years preferred.)
- Ability to perform patient evaluations, develop and update plans of treatment, and provide documentation in accordance with state and agency requirements.
- Familiarity with electronic documentation systems and basic computer navigation (email, internet, user accounts).
Preferred Qualifications
- 2-3 years of physical therapy experience preferred.
- Experience supervising PTAs and home health aides preferred.
- Previous travel therapy experience preferred.
