Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active California physical therapy license is required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience providing care in home health settings is preferred.
- Ability to perform evaluations and develop individualized treatment plans for adult patients with musculoskeletal and chronic conditions.
- Reliable transportation and ability to travel locally to patient homes in the Santa Rosa region.
Preferred Qualifications
- Previous travel or per diem experience in home health.
- Experience with documentation platforms commonly used in home health agencies.
- Advanced certifications relevant to geriatrics or orthopedics preferred.
Assignment Details
- Shift Days: 5 days per week
- Region: Santa Rosa area; clinician may be expected to travel between patient homes within the region.
- Two Physical Therapists needed for the Santa Rosa region.
