Minimum Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Indiana physical therapy license required prior to the start of assignment.
- BLS certification from AHA or ARC may be required.
- No new graduates. Clinical experience in outpatient physical therapy required.
- Competence in comprehensive patient evaluation, goal writing, treatment planning, and documentation in compliance with regulatory and payer requirements.
- Ability to complete timely, accurate medical record documentation including functional quality indicators and letters of medical necessity.
Preferred Qualifications
- Experience supervising or precepting students and providing a structured learning environment.
- Experience with safe patient handling practices and materials handling protocols.
- Demonstrated engagement in continuing education and professional development activities.
Assignment Details
