Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification May Be Required from AHA or ARC
- Home Health Experience
- Physical Therapist License in MI
- Doctoral
Assignment Details
- Shift Hours: 8:00 AM - 4:30 PM
- Shift Days: Monday - Friday
- No on-call requirement
- Coverage area includes all of Delta County and portions of Dickinson and Menominee Counties within an approximate 1-hour radius of Escanaba
- Responsibilities include physical therapy admissions, add-on evaluations, reassessments, discharges, creating individualized plans of care, supervising and collaborating with PTA, and participating in weekly case conferences and monthly staff meetings
