Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Kentucky Physical Therapy license required to start the assignment. Out-of-state license with ability to obtain KY license prior to start is acceptable if client permits.
- Current BLS certification from AHA or ARC may be required.
- Experience in outpatient neuro setting preferred; candidates with clinical rotation experience in outpatient neuro will be considered.
- Experience with comprehensive evaluation, goal writing, and interdisciplinary team collaboration in an acute or post-acute setting.
- Competency with documentation, including daily notes, functional quality indicators, letters of medical necessity, discharge documentation, and charge entry per facility and payer requirements.
- Ability to participate in student supervision and support professional development activities.
- Adherence to state practice act, facility policies, and safety protocols including safe patient handling and infectious disease precautions.
- Preferred: prior experience in LTACH or inpatient rehab settings. This is preferred, not required.
Assignment Details
- Facility Type: Long Term Acute Care Hospital.
- Shift: Day (5x8).
- Duration: 13 week assignment.
- Caseload: Outpatient neuro-focused caseload within an inpatient rehab/outpatient hub model.
- Clinical Responsibilities: Comprehensive evaluation, development of treatment plans and discharge-focused goals, equipment trialing and ordering, patient and family training, and interdisciplinary collaboration.
- Documentation: Timely and accurate medical record documentation per facility, payer, and regulatory requirements.
