Physical Therapist - Home Health (Fort Mill, SC)
Provide skilled physical therapy services to home health patients across Fort Mill, SC. The clinician will evaluate patient needs, develop and implement individualized plans of care, document progress, and collaborate with physicians and interdisciplinary teams. Responsibilities include therapeutic treatment for neuromuscular, neurological, and orthopedic conditions, patient and family education, equipment recommendations and training, application of infection control practices, and supervision of ancillary staff when applicable.
Minimum Qualifications
- Current state license as a Physical Therapist in good standing.
- Bachelor of Science, Master’s, or Doctoral degree in Physical Therapy from an APTA-approved program.
- Proof of current CPR certification.
- Current driver’s license, reliable transportation, and vehicle insurance.
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting.
- Ability to perform prolonged standing, walking, lifting, bending, pulling, and pushing as required for home visits.
- Must read, write, and speak fluent English.
Preferred Qualifications
- Previous home care experience.
- Knowledge of durable medical equipment and ordering/adapting equipment for home environments.
- Demonstrated commitment to continuing education and professional growth.
Assignment Details
- Must be able to travel locally and perform home visits within the service area.
