Minimum Requirements:
- Must have State License to Apply
- 6 months Pharmacist experience preferred
- EPIC experience
- Medication reconciliation for discharge medication list
- Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy
- Communication skills for discharge counseling with patients/families
- Clinical knowledge of antimicrobial stewardship
- Interpretation of culture results for recommendations
- Knowledge of UTI, pneumonia, and SSTI for recommendations
- Pharmacokinetic dosing management/monitoring
- Anticoagulation dosing management/monitoring
