Our client is looking for Case Managers across the state of California. This role will provide case management support for members enrolled in client health plans to promote timely healthcare delivery. You will be conducting comprehensive assessments for long term care patients and then helping coordinate the appropriate visits, contacts, and referrals to other programs to address the members' needs. You will be supporting specifically unhomed and elderly patients, including some disability. Additionally, you will document all communications and care updates accurately and ensure all reporting is handled within the given requirements. The role will be roughly 75% field visits within their region with 25% remote.
Long term care or managed care experience
Bilingual in Spanish
2 years minimum of relevant experience
Must be willing to travel within their region
