Infection Prevention & Wound Care Supervisor
Educational Requirements
-
Graduation from an accredited School of Nursing.
Experience Required
-
Minimum of 1 year RN supervisory experience in a Long-Term Care setting.

Strong leadership and excellent clinical skills required.
-
Experience in Infection Control in a sub-acute or long-term care setting preferred.
-
At least one year of experience in wound/skin care preferred.
Licensure & Certifications
-
Current, unencumbered, active RN license in New York State.
-
Current CPR and IV certification.
-
Infection Control Certification (required by 11/2019).
-
Wound Care Certification preferred (AAWM or WOCN).
Essential Functions
Supervisory & Clinical Leadership
-
Coordinate the administration and clinical activities for the nursing department on assigned shift, in accordance with regulations governing long-term care.

Ensure new admissions are completed thoroughly with all pertinent assessments.

Address emergent resident needs, anticipate changes in condition, and make appropriate clinical decisions.
-
Serve as a clinical resource and provide guidance to staff in decision-making; delegate responsibilities appropriately.
-
Conduct frequent rounds on nursing units to ensure care standards are met and residents’ needs are addressed.

Monitor staff documentation for accuracy, completeness, and compliance; follow up on concerns with staff and report to the Director of Nursing.

Participate in the development, implementation, and review of resident care plans.

Assist with staff performance evaluations, coaching, and disciplinary actions as needed.

Participate in state and federal surveys and inspections, representing the facility as required.
Infection Prevention & Control
-
Plan, implement, monitor, and evaluate the Infection Prevention & Control Program in compliance with CDC, OSHA, state, and federal guidelines.

Conduct infection surveillance and investigations; maintain accurate infection records for residents and staff.

Lead infection control rounds to monitor sanitation, hand hygiene, and isolation precautions.

Provide education and training on infection prevention policies and practices to all staff.

Collaborate with department heads to ensure compliance with universal and transmission-based precautions.

Report communicable diseases to state and local health authorities.
-
Serve as chair or member of the Infection Prevention & Control Committee, preparing reports and recommendations.
Wound & Skin Care
-
Conduct wound assessments and develop individualized treatment plans.
-
Provide wound care consultations to staff, residents, and families.
-
Coordinate interdisciplinary wound rounds and facilitate physician consultations as needed.
-
Serve as a resource for wound care product selection, treatment methods, and pressure ulcer prevention.

Provide wound care education and competency training for nursing staff.
-
Participate in quality improvement initiatives related to skin integrity and wound care outcomes.

Contribute to discharge planning for residents with wound and skin care needs.
Administrative Functions
-
Prepare monthly and quarterly reports on infection control and wound care outcomes for the Administrator and Director of Nursing.

Participate in policy and procedure review and revisions related to nursing, infection control, and wound care.

Assist with product and supply evaluation for cost-effective, evidence-based care.
-
Maintain accurate records and ensure compliance with regulatory documentation standards.
Additional Duties & Expectations
-
Report to work on time, in compliance with facility dress code, and wear ID badge while on duty.

Provide timely notification of lateness or absence.
-
Adjust scheduled time off as needed to meet departmental needs.
-
Perform other duties as assigned by the Director of Nursing.
