Night Shift - 8\:00 PM - 8\:00 AM
Adheres to all Health First, Inc. and Hospice of Health First Policies and Procedures.
Provides nursing care and prescribed treatments for patients in their home, according to the physician's plan of care as supervised by RN.
Reports alterations to the plan of care as indicated by changes in observed condition of patient to the appropriate RN.
Assures the safety of the treatment procedures, patient, and environment at all times.
Assists with the orientation process of new associates
Complies with State and Federal Regulations, JCAHO standards in reference to delivery of Hospice Services
Actively participates in Quality Improvement processes.
Accepts assignment willingly regardless of geographic territory.
Attends Staff Meetings - (80% Minimum)
Assists in organization of patient care, scheduling and documentation reviews as requested by supervisor.
Communicates to Case Manager and/or Supervisor any adverse findings to patient condition and documents findings in the medical record.
Notifies Supervisor of any change in their projected scheduled visits and number of hours worked to not exceed 40 hours a week
Available for weekend coverage/assignments as needed
Demonstrates effective administration of medication and IV therapy
Meets or exceeds Patient Satisfaction goals as established every year.
Performs other related duties as assigned or requested.
Ensures appropriate utilization of visits and services for care under the direction of the RN as noted through proper documentation completed at time of the visit.
Utilizes supplies in a cost-effective manner.
Meets productivity requirements
Graduate of an accredited school for LPN Nursing.
Minimum of 3 years acute care experience.
Hospice/Home Care experience preferred
Criminal History Screening.
Current Florida Drivers License, automobile insurance and reliable transportation.
Must hold current AHA BLS Healthcare Provider Completion Card prior to start date and maintain.
Current licensure in the State of Florida as a LPN.
Good communication skills.
Certification of 30-hr, State approved IV therapy education, which includes four hrs of Central Line education and Clinical Practicum preferred
Possible exposure to bio-hazardous waste, chemical substances, and unknown diseases.
Care is provided in a variety of home environments with variable temperatures.
Exposure to animals in the home.
Driving, traffic, road hazards possible.
Ability to move in crowded conditions.
Ability to operate automobile in variable conditions.
Visual acuity and hand-eye coordination to perform clinical tasks.
Must be able to lift, turn, pull, or ambulate with patients up to 20 times daily.
Must be able to lift up to 40 pounds unassisted up to 6 times daily.
Must be able to speak and understand the English language.
Must be able to stand, walk, bend, and stoop for 90% of workday.
Must be able to tolerate a minimum of 3-5 hours of computer time a day
Ability to interact with other professionals, families, supervisor and patients.
Ability to work in stressful environments/situations.
Ability to decide concerning appropriate patient care.
Ability to respond appropriately to life threatening situations.
Ability to assess, interpret data and educate patients and families of various ages and levels of understanding.
Ability to work independently within time constraints and concentrate under multiple demands
