We are seeking a dedicated and compassionate Registered Nurse - Field UAS Assessments to provide essential home healthcare services in the New York Metropolitan area. Duties
Conduct comprehensive UAS-NY assessments to determine eligibility and care needs, ensuring our patients receive the best community-based long-term care services.
Collaborate with interdisciplinary teams to promote our PACE program, educating potential participants and referral sources about the benefits of our services.
Review and maintain accurate documentation in EMR systems, while addressing any significant changes in patient health.
Engage in quality improvement initiatives to enhance patient care and outcomes, while adhering to CMS/PACE guidelines.
Qualifications
Current nursing license in the state of practice.
Experience in medical-surgical nursing or working within a hospital environment is preferred.
Familiarity with emergency medicine protocols and practices is advantageous.
Strong knowledge of anatomy and aseptic techniques is essential.
Ability to work effectively with individuals with disabilities, demonstrating empathy and patience.
Excellent communication skills, both verbal and written, for effective patient interaction and documentation.
Ability to work independently while adhering to established protocols and guidelines.
Pay Range:
$100 - $160 per assessment
