This position is actively involved with psychosocial screening, facilitating the safe transition of care to home of every child admitted to the pediatric facility. Administrative functions include the planning, organization, and implementation of the policies & procedures and operation of the Social Service department in accordance with the federal, state and local standards, guidelines, and regulations. The goal is to meet the medical and psychosocial needs of the pediatric residents while in the facility and after discharge.
Responsibilities & Duties:

Participate in policy & procedure making/review/implementation.

Resident assessment/screening, initiation/implementation and evaluation of care plan.

Resident & family communication, education and discharge planning.

Facilitate resident & family access to healthcare, medical needs, different insurances, community resources and public assistance.

Maintain effective relationship & communication with the different interdisciplinary team and other facility stakeholders.

Collaboration and working closely with Ditmas' other department and services such as the dialysis department.

Will provide social services to the pediatric residents. Knowledge of pertinent services, agencies, and regulations related to pediatric healthcare i.e Child Protective Services, child abuse prevention, etc.

Participation in the facility surveys/inspections.
Qualifications & Skills:

Master's and Bachelors degree in social work, sociology, or special education is a must.
