The Disposition Planner is responsible for disposition planning, psychosocial and functional assessments, patient advocacy and
education for patient and family. The Disposition Planner will provide resources, information and referrals that benefit adjustmentto illness, hospitalization and recovery throughout the continuum of care. The Disposition Planner will provide appropriate communication/intervention based on specific age groups.


Hours: Friday through Monday for 10 hour shifts. 8am to 6pm.


Required Minimum Education: Required Minimum Education: Masters Degree in Social Work or Graduate of an
Accredited School of Nursing.
Required Minimum Certification: LMSW (or eligible) in the state of Georgia or Licensed Registered Nurse
in the state of Georgia. Certification in Case Management is desired.
Required Minimum Experience: Minimum 3 years of clinical practice/experience in general medicalsurgical
and/or disposition planning.
Required Minimum Skills: Organizational and professional communication skills a must. Ability to act as a liaison
with multidisciplinary team members. Knowledge of case management. Working knowledge of community resources.
Good disposition planning and assessment skills. Good prioritization skills
