Assist residents with medications under the direct supervision and delegation of the Director of Patient/Health Services. Provides assistance to Residents, as assigned, who need help with ADLs in a discreet and private manner.
DUTIES AND RESPONSIBILITIES
• Assist with the self-administration of medication as delegated by the Director of Patient/Health Services.
JOB DESCRIPTION – MEDICATION AIDE
• Assist with Self Administration of Medication can only be delegated to the Attendant if the Home Health Aide has completed and passed a Medication Assistance Training Program, and if the delegating Director of Patient Health Services decides that the Home Health Aide is competent to execute the duty being delegated
• Checks for expiration dates on medications.
• Reorders medications through resident’s chosen pharmacy.
• Reads communication Logbook and checks with supervisor regarding any changes in Resident medication instructions.
• Checks Medication Assistance Record (MAR) to be sure all necessary changes are reflected and advise Director of Patient/Health Services or Designee.
• Keeps medication carts neat and clean.
• Replenishes supplies on medication cart as needed.
• Provides medication reminders and assistance.
• Performs MAR Review at the beginning of each month and when necessary.
• Advises Director of Patient/Health Services or Designee when updated Rx is needed.
• Establishes re-order schedule for Residents who do not use house pharmacy.
• Advises Director of Patient/Health Services or Designee of resident refusals of any medications at any time.
• Maintains accurate count of all narcotics upon start and end of shift.
• Checks for accuracy of refrigerated medication at the start of 7:00am-3:00pm shift. Temperature of the refrigerator should be maintained at 36-40 degrees Fahrenheit.
• Assists with activities of daily living as assigned or needed by residents as outlined in individual service plans including, but not limited to, bathing, dressing, toileting, oral hygiene and nail care.
• Engages residents in meaningful activities in order to enhance their quality of life.
• Assist with and follows directions as requested by the Director of Patient Health Services/LPN Supervisor.
• Reports and/or records all pertinent medical or social changes that are observed to the Director of Patient Health Services or LPN Supervisor
