Position Summary:
Blue Palms Health is seeking a full-time Occupational Therapist to provide skilled OT services in our in-house subacute rehabilitation unit. You will help patients regain independence, improve daily living skills, and safely transition through care.

Key Responsibilities:

Perform comprehensive occupational therapy evaluations for subacute patients

Develop, implement, and update individualized treatment plans per physician orders

Provide skilled interventions for ADLs, mobility, strength, cognition, and safety

Collaborate with an interdisciplinary team including physicians, nursing, PT, SLP, and case management

Participate in care planning and discharge processes

Complete timely, accurate documentation per facility and regulatory requirements

Educate patients and families on therapeutic techniques, adaptive equipment, and safety strategies
Qualifications:

Current Occupational Therapist license (or eligibility) in good standing

Graduate of an accredited Occupational Therapy program

Subacute or skilled nursing rehabilitation experience preferred but not required

Strong clinical judgment, communication, and documentation skills

Ability to work effectively in a fast-paced, team-oriented environment
