The San Jose Mission Clinic has provided free healthcare services to uninsured and underserved community members for more than two decades. Located on the San Jose Mission campus, the clinic offers primary care, chronic disease management, and specialty services through the dedication of its staff and volunteer physicians.
As a nurse in this role, you'll have the opportunity to make a meaningful impact by delivering compassionate care to those who need it most while serving a mission-driven community.
Nurse Care Coordinator, RN key responsibilities:
- Responsible for medical intake procedures, maintaining daily patient flow, assisting with organization of patient care services, & laboratory and phlebotomy services, and maintaining clinic compliance with governmental agency standards.
- As part of a multidisciplinary team coordinates the provision of all medical services.
- Assures appropriateness of patient care, facilitates case management, encourages appropriate utilization of resources and timely specialty consults.
- Provides education to other team members.
- Perform other duties as assigned.
Minimum Qualifications:
- Required - Associates - Nursing; Or - Diploma - Nursing
- Required - RN (Registered Nurse)
- 3 years of Clinical Related Experience
Highly Preferred Skills:
- Proficient in English and Spanish
