The Assessment Worker/Family Resource Coordinator responsibilities include:
- Reviewing hospital or clinic records and interviewing families, prenatally or immediately after birth, to determine families eligible for services through the Healthy Families/Healthy Start Program.
- Conducting comprehensive assessments within the hospital using standardized protocols and instruments and design individualized interventions for families designed to improve pregnancy and childhood health outcomes.
- Performing other duties as assigned.
Position details:
- Location: St Josephs Hospital South - Riverview Florida AND St Joseph's Women's Hospital - Tampa Florida
- Status: Part time, 28 hours per week
- Schedule: Saturday & Sunday 7:00 AM – 5:30 PM - St Joseph's Hospital South
Another weekday(flexible) - 8 hour shift - St Joseph's Women's Hospital
- Weekend Requirement: Every
- On Call: No
Education/Experience:
- Bachelors Degree in Related field with 2 years’ experience working with women, children or Family Services OR
- High School Diploma with 3 years’ experience with Healthy Families program
