Nurse Practitioner (Skilled Nursing)

Major Responsibilties 

  • As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday.

  • You will collaborate with the facility’s interdisciplinary team and physicians to reduce hospitilizations on weekends. 

  • You will work with the entire facility’s patient population - short and long-term residents with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians

  • Evaluation of new-admission visits

  • Coordination of discharge visits

  • Evaluations for patients exhibiting changes in condition

  • As a SNF-ist, you will provide types of care, including but not limited to:

  • Advanced Care Planning

  • Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease

  • Prescribe all necessary medications and treatments

  • Perform comprehensive physical examinations of patients

  • Develop and implement patient management policies and procedures

  • Facilitate referrals to other healthcare professionals and medical facilities

  • Communicate with attending physicians and/or specialists regarding patient care

Qualifications

  • Advanced Practice Licensure (APRN) with a Texas license

  • Must have an active license and DEA number. 

  • Previous experience in nursing or other medical fields.

  • Ability to build rapport with patients and their loved ones.

  • Ability to thrive in a fast-paced environment.

  • Excellent written and verbal communication skills.

  • Strong leadership experience with emphasis on teamwork in a healthcare setting.

  • Master’s degree required
