Overview
The RN UAS Assessment Nurse is a field based position for the purpose of conducting UAS initial and reassessment visits. The RN UAS Assessment Nurse will be responsible for the completion of in-home assessments, tasking tools, and all other required paperwork or electronically submitted documentation related to the completion of UAS assessments. Reports to UAS Program Director.
Responsibilities
- Schedule and conduct face to face comprehensive clinical assessments for potential and current Adult Day Care members via UAS assessments according to state guidelines, policies, procedures, and protocols.
- Visit MLTC members receiving long term care.
- Perform assessments in the home setting.
- Create service plans and determine eligibility and continuing program services via the UAS.
- Conducts comprehensive assessment of all aspects of potential new members condition - clinical, social, and environmental to determine the level of services needed to appropriately keep member safe in the community.
- Gathers clinical information from various sources to address all conditions, and follows appropriate guideline, policy, procedure and clinical judgment in completing assessment requirements.
- Conduct needs assessment by utilizing UAS-NY/DOH tool to determine consumers qualifications and appropriateness for Long Term Care Services.
- Communicates with consumers, families, providers, and other parties as needed to complete accurate comprehensive assessment.
- Understands the needs of vulnerable populations who have chronic or complex bio-psychosocial needs.
- Consult with health care providers and review medical records and medications to support assessment outcomes.
- Conducts all assessment functions in compliance with regulatory and company policies and procedures.
Qualifications
Education
Associate's Degree (Bachelor's Preferred)
Work Experience
1 year prior experience preferred.
