Overview
Provides supportive services to residents and families following admission to the nursing home. Responsible for all aspects of discharge planning.
Responsibilities
Essential Functions
Assess each residents leisure function and interests upon admission and repeat, as needed.
Develop care plans based on assessment findings and implement Therapeutic Recreation Programming to meet the specific goals laid out in the care plan.
Evaluate the effectiveness of Recreation Programs in both meeting the interests of the residents, and achieving care plan goals.
Document resident progress, as required by regulation and department policy.
Maintain attendance records for all Therapeutic Recreation programs and statistical data detailing each residents participation in Recreation programs.
Participate in all Corporate, Facility, and Departmental Quality Assurance initiatives.
Plan and implement special events and community outings.
Attend all inservices, staff meetings, care plan meetings, and and partipates in interdepartmental committees as assigned.
Setup and operate all necessary equipment for Recreation programs and transport residents to and from Recreation programs.
Supervise volunteers.
Ensure that adequate leisure supplies are available and inform supervisor of supplies that need to be ordered.
Develop and post monthly calendar of Recreation activities on resident unit and at each resident's bedside.

Experience

Minimum Knowledge
Requires extensive knowledge in several specialized disciplines and ability to integrate critical information from many diverse areas. Requires extensive theoretical and practical knowledge.
Language Ability
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Education Requirements
Master of social work degree from an accredited university; or bachelor degree from an accredited university and at least two years experience in the field of social work.
License Requirements
LMSW preferred.
