Overview
Will perform the Job descriptions of a Specialty Hospital Social Worker.
Responsibilities
Two years of clinical experience, at least one with developmentally disabled.
1. In-depth knowledge and understanding of mental retardation and developmental disability.
2. Knowledge of behavioral and cognitive behavioral treatment.
3. Reasonable understanding of OPWDD and Medicaid regulations, including the specific requirements for profoundly delayed inpatients.
4. Knowledge of community resources for the DD population to facilitate referrals and communication with other agencies.
5. Understanding of the needs of the profoundly impaired inpatients on Specialty Hospital, and community and in-house resources for the same population.
Skill Requirements
1. Excellent verbal and written communication skills.
2. Ability to write treatment plans that contain observable, measurable, and consumer-appropriate goals.
3. Skill in writing progress notes in standard format that includes subjective and objective observations, a goal-oriented plan, relevant interventions used, and an assessment of each session.
4. Ability to establish a therapeutic alliance with consumers and families.
5. Sufficiently skilled in various therapeutic modalities to be flexible in choosing the appropriate treatment across patients and over the course of treatment as consumer's needs and abilities change.
6. Basic computer skills, including ability to learn to use medical manager system and electronic medical record.
7. Able to work as a case manager, when needed.

Essential Functions
1. Provides Five Star Customer Service to all internal and external customers 100% of the time.
2. Contributes to the overall performance in attaining the four following results: High quality care, Provider of choice, employer of choice, Financial success.
3. Provides individual psychotherapy to consumers.
4. Includes families and other caretakers or collaterals in psychotherapy, as appropriate.
5. Develops and runs therapy groups.
6. Develops and implements specialized workshops for consumers.
7. Provides consultation to other DDC staff on issues related to social work and case management.
8. Participates in case conferences regarding consumers care and treatment.
9. Interfaces with outside agencies such as schools and day treament programs to coordinate care, gather information, and facilitate interventions for consumers in those settings.
10. Participates in networking and outreach to market DDC services to the community.
11. Prepares daily statistics.
12. Develops a treatment plan for each consumer in treatment and updates it every six months, or as changes occur.
13. Writes a summary of each ongoing treatment every six months.
14. Conducts psychosocial assessments and prepares written reports of same.
15. Assists treatment coordinators in following through on evaluation recommendations.
16. Attends and participates in relevant in-service training and continuing education experiences.
17. Participates in weekly supervision.
18. Gathers, analyzes, and reports on data for quality improvement audits.
19. Attends outside meetings, as needed, for outreach, networking and marketing of the clinic.
20. Registers inpatients with Board of Education for onsite education.
21. Identifies and activates guardianship application procedures, where appropriate.
22. Attends quarterly case conferences on Specialty Hospital patients.
23. Invites family, law guardians, and involved agencies to quarterly case conferences.
24. Serves as liaison between Specialty Hospital patients, their families, legal guardians, lawyers, schools, outside agencies, and vendors.
25. Reviews all assigned Specialty Hospital records at least once a month.
26. Attends outside conferences for Specialty patients with ACS and protected medical foster care agencies, and at schools for those who attend school outside the facility, as needed.
27. Provides consultation to other TCC staff regarding the social work/case management needs of Specialty Hospital patients.
28. Reviews advanced directives upon admission and at appropriate intervals with Specialty Hospital families.
Qualifications
Excellent in communication with strong organizational and clinical assessment skills.
