Mission
THE MISSION of Archcare is to foster and provide faith-based holistic care to frail and vulnerable people unable to fully care for themselves. Through shared commitments, the System seeks to improve the quality of the lives of those individuals and their families.

Responsibilities
Essential Functions
Collaborate with patient or representative and interdisciplinary team to ensure seamless transfer to the facility, including the discharge plan.
Acts as the single point of decision making for admissions, collaboration as indicated
Completes a pre-admission assessment identifying medical, social issues and financial coverage.
Analyze the participants needs and match with facility services
Participate in quality improvement processes.
Conduct on-site visits at the acute care setting, facility and resident home as needed.
Contribute in the development of policies and procedures as needed.
Act as an initial contact for discharge and transfer of patients from area hospitals and other care settings to the Program.
Make rounds with case managers and or other hospital staff as requested.
Act as a liaison between physician and patient (or representative) and between patient and referral sources and the Program.
Review appropriateness of patient care information. Facilitate conveyance of accurate discharge information/physician orders.
Communicate regularly with hospital and extended care/post acute facility social services to ensure smooth communications and avoid duplication of efforts. As part of this on-going communication, maintain current daily census, acuity and service availability information.
Trend, analyze and report data and variances related to seamless patient transfers to the Program. Trend, analyze and report patterns related to services provided by either the hospital or the facility for action and improvement as requested.
Assist in the reduction of hospital readmission rates by reporting the most current, complete and relevant medical/clinical information to the facility.
Perform other duties as assigned within the scope of the position.

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.

Experience Requirements
a minimum 3 years experience in case management for post-acute, Medicare or long-term care (preferred)
Have 1 year of experience with a frail or elderly population
