Overview
The Physical Therapist is responsible for the planning, provision and coordination of physical therapy to patients in a home setting in compliance with applicable laws, regulations and Agency policies.
Responsibilities
- Performs initial and ongoing physical therapy evaluation according to APTA standards of practice and New York State Law and Rehabilitation Department guidelines.
- Provides physical therapy services for the patient, as ordered by the physician.
- Develops a plan of care for the patient, in collaboration with the nurse, identifying short and long term goals and patient and caregiver involvement.
- Intervenes in the restorative health care plan upon a request for evaluation and treatment of a physical disability and submits written initial evaluation for inclusion in patients record.
- Performs clinical tests to discover functional limitations and the potential for restoration and develops a plan of care with restorative and maintenance goals to meet identified needs.
- Applies physical therapy procedures to achieve identified goals, (i.e., therapeutic exercises, functional training).
- Instructs patient, family and other health team personnel in the use of assistive modalities and appropriate physical therapy procedures.
- Observes, records and reports to the physician and the nurse, the patient reaction to treatment and any changes in the patient condition.
- Works with the health team to shorten the patient recovery from illness and incorporates the management of residual disability.
- Re-evaluates patients periodically to determine progress and documents appropriately; every 60 days provides a written summary and evaluation report for the physician and the record and every 120 days a reassessment of the patient.
- Maintains communication with physician and other personnel involved in the patient care, through written reports, attendance at meetings and confirmed telephone calls.
- Submits timely clinical records on the patient with the type of treatment given, modality used and patient response.
- Fits assistive devices for self care and ambulation; trains patient in the use of devices and inspects for function.
- Participates in in-service training programs for all staff.
- Performs other related duties as required.
- Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan.
Qualifications
A current New York State license to practice as a Physical Therapist.
Current Drivers License, required insurance and car available for work as required.
Good organizational, observation, communication and judgment skills.
Is self-directed with ability to work with supervisor.
Is flexible and cooperative in fulfilling role obligations
