Overview
Opened in 1973, ArchCare at Ferncliff Nursing Home is located on 36 beautiful acres of the former Astor family estate just outside the historic village of Rhinebeck. Our goal is to provide the best possible experience through holistic care that addresses not only the residents' physical needs, but their social, psychological, emotional and spiritual needs as well.
The License Practical Nurse (LPN), under the supervision of a Registered Professional Nurse (RN), provides direct resident care as delegated, and assist the RN in the delivery of resident care utilizing the nursing process as appropriate to the geriatric resident. Participants in resident care services as directed by supervisor in accordance with in-service training.
Responsibilities
- Carries out procedures and treatments according to signed physician orders and nursing plan of care.
- Participates in the development, implementation and on-going nursing care plans and nursing care activities, including participation in case conferences.
- Provides instructions to patients, families, and/or significant others in accordance with the registered professional nurses plan of care.
- Identifies and reports changes in the patients condition and/or family situation to the physician and registered professional nurse.
- Records information concisely, accurately and completely according to Agency policies, procedures and guidelines, and submits documentation in a timely manner in accord with Agency policies.
- Understands and interprets Agency policies and procedures.
- Identifies and utilizes appropriate channels to communicate with appropriate persons, including but not limited to, the RNs assigned to the patients case, management, and physicians.
- Participates in performance improvement activities.
- Performs other duties as assigned by the RN.
Qualifications
- Current license and registration to practice as a Licensed Practical Nurse in New York State.
- Graduate of an accredited practical nursing program
- One year clinical experience in a medical/surgical setting.
