Overview
The License Practical Nurse (LPN), under the supervision of a Registered Professional Nurse (RN) , provides direct resident care as delegated , and assist the RN in the delivery of resident care utilizing the nursing process as appropriate to the geriatric resident . Participants in resident care services as directed by supervisor in accordance with in-service training.
Responsibilities
Responds to resident call bell and refer resident and family request to appropriate health care team member.
Collects data on assigned residents to assist the RN in assessment of the resident, and development of the plan of care.
Assist in the planning, implementation, and evaluation of plan of care after establishing with the RN resident goals based on identified problems or needs.
Evaluate and modify plan of care in collaboration with the RN when changes in the resident''s condition occur.
Perform accurate nursing documentation of observations, nursing interventions and resident response to treatments in the resident record in accordance with established standards, policies and procedures.
Assist in resident /facility instruction in accordance with established standards.
Qualifications
Education Requirements
Graduate of a state Board of Nursing Approved Licensed Practical Nursing Program.
Experience Requirements
One to three years in long term care preferred.
License Requirements
Currently licensed as a licensed Practical Nurse New York State.
