Overview
The Enrollment Nurse (EN) is responsible for conducting in home assessments for prospective enrollees utilizing clinical nursing expertise and critical thinking skills. As needed may need to complete assessments in settings other than the home, for ex: skilled nursing facilities. The EN determines clinical eligibility and whether the prospect meets the criteria for PACE (Program for all-inclusive care for the elderly) benefits.
In this role, you will collaborate with an interdisciplinary team (IDT) of Primary Care Providers, Nurses, Social workers, Rehab staff, Nutritionist, and Adult Day Health Care Staff, etc.The PACE program supports frail elderly in the community that would otherwise be in a nursing home. The interdisciplinary team works under one roof which allows for the team to collaborate, and care plan for the needs of the participant.
Responsibilities
- Enrollment Nurse conducts face to face in depth nursing assessments, and documents compliantly.
- Enrollment Nurse utilizes a skillful approach for assessing, coordinating and evaluating enrollees needs, and development of the initial prospective plan of care
- The Enrollment Nurse will be expected to perform assessments on multiple participants or potential enrollees in a timely manner
- Explains PACE benefits, policies, and procedures for potential enrollees, and current participants, providers, and community-based agencies
Qualifications
Graduate of an accredited registered nurse program.
Registered Nurse with current valid nursing license in the state of New York
One year of experience with a frail or elderly population.
Demonstrate ability and initiative to work independently in assessing the patient's eligibility for PACE and demonstrate good written documentation skills
Experience working with a frail adult or elderly population with the ability to determine appropriate care plans and services for frail population
Excellent interpersonal, verbal, and written communication, time management, organizational and customer service skills.
Demonstrate accountability and accuracy in assigned tasks along with effective problem solving ability
Proficient in Microsoft Office (Word, Outlook), and Electronic Medical Record
