Overview











The Director of Patient Services (DPS) is responsible for the overall clinical direction of the Assisted Living Program and Home Care Services. The Clinical director established, implements and evaluates goals and objectives for Assisted Living Program and Home Care Services that meet and promote the standards of quality and contribute to the total organization and philosophy. The DPS will assure safe, effective and efficient home care is provided by the home care team consisting of RN''s, LPN''s, physical therapists, occupational therapists, speech therapists, care managers (social workers) and aides. Provide support for aide supervision. The DPS will assess, plan, implement, and evaluate effectiveness of nursing care delivered and communicate information appropriately to physician and the interdisciplinary team. The DPS assure adherence to corporate policies, procedures and all regulatory guidelines for clinical staff.





Responsibilities











Develops and implements an effective and efficient service delivery model by ensuring federal and state regulatory compliance for patients, residents and staff. Identifies, recognizes, matches and utilizes staff strengths and skills with departmental demands. Completes, maintains and adheres to departmental staffing plan.









Assist in the preparation, implementation and management of the annual budget to meet expectations









Maintains records according to Department of Health Standards for Adult Care Facilities Assisted Living Program (ALP).









Maintain current knowledge and adhere to all third party agency regulations









Maintain appropriate quality documentation of all resident services provided, according to program procedures policy and procedures and regulations.









Maintains client confidentiality and adherence to HIPAA requirements at all times.









Completes all required documentation in a timely manner and per requirements.









Conduct appropriate nursing assessment as required by the Licensed Agency procedures to facilitate admissions and care planning process for residents









Effectively support corporate goals and initiatives through personal goal setting, communications with staff and monitoring progress.









Attend and participate in all required meetings. Participate on relevant committees/task teams.









Establish and maintain cooperative working relationships with other program staff, outside agencies, and provider hospitals.









Participate in community planning related to nursing and health needs. Offer expertise through formal or informal educational opportunities.









Participate in intra/inter-departmental meetings and intra-agency meetings, conferences and seminars as required.









Maintains records according to Department of Health Standards for Adult Care Facilities/Assisted Living Program and Licensed Home Care Service Agency (LHCSA) policy and procedures.









Utilizes management and administrative skills to accomplish ALP facility goals.









Demonstrates working knowledge of NYSDOH and regulatory agencies mandates /standards.









Fosters problem solving abilities to staff.









Assist with the development, implementation and evaluation of Quality Assurance









Analyze QA indicators and initiate remedial action as required. Maintain threshold of errors below target level. .









Effectively support Quality Assurance initiatives through data gathering, analysis and actively participating in Quality Assurance meetings.









Assist in resolution strategies and implementation of plans to address issues/problems with residents and staff as they arise.









Monitor delivery of resident/client care to assure quality expectations are met. Maintain an effective resident complaint/issue resolution system with Administrator for areas of responsibility.









Conduct periodic nursing assessments and evaluations for each resident and routine nursing care for ongoing or episodic illness according to the plan developed by the team.









Counsel resident families regarding measures to maximize independence as a solution to physical, emotional and environmental health problems.









Make referrals to other services when resident conditions or situations require the service of other professional disciplines or products/services of outside companies or agencies.









Participate in the development and revision of the resident plan of care as a member of the interdisciplinary team.









Provide instruction and demonstrate nursing techniques to resident and care givers.









Provide on-call coverage on a rotating basis with other nursing staff to ensure consistency in the coordination and scheduling of patient care.









Coordinate skilled nursing care and prescribed treatment to the residents in ALP per mandates and regulations









Actively participate as part of interdisciplinary team with suggestions for interventions and recommendations for revised treatment plans to address resident concerns and issues









Maintain communication with other team members to discuss progress and coordinates resident care efforts.









Provides ongoing case management services by conducting home visits, monitoring delivery of services and quality of care, and reassessing client needs including ongoing eligibility for services; tracks client progress and well being, evaluates case plan effectiveness, and effectively manages resources; prevents conflicting case plans and duplication of services.









Shares current knowledge and up to dated clinical expertise with staff through formal and informal education.









Provides for the on-going evaluation and achievement of RN, LPN, HHA and PCA clinical competencies.









Responsible for the supervision of HHAs per licensed agency regulations in conjunction with the ALP.









Coordinate skilled nursing care and prescribed treatment to the residents in ALP per mandates and regulations.









Maintain communication with other team members to discuss progress and coordinate resident care efforts.

Thorough knowledge of regulatory requirements of DOH and all pertinent agencies.









Computer competency in MS Office Suite or equivalent.





Qualifications




Education Requirements








Bachelor''s degree.






Education Desired








Bachelor''s degree or Master''s degree in Nursing from an accredited college or university preferred.






Experience Requirements








Current NYS license and registration in good standing as a professional nurse.
