Overview
The Clinical Liaison will engage in assessing and evaluating patients in acute care settings that meet the criteria for admission to an ArchCare facility. The Clinical Liaison will evaluate the medical and clinical healthcare needs and assist with the coordination and planning of all possible referrals made to all ArchCare facilities.
Responsibilities
Maintain confidentiality of patients.
Maintain a positive, effective, and professional working relationship with referring hospital staff.
Prepare clear and concise reports as needed.
Respond to requests and inquiries from the referring hospital promptly (15 30 minutes).
Assess and evaluate patients needs for services, treatment, and medication.
Qualifications
RN, BSN, preferred. All required licenses must be maintained in an active status without suspension or revocation throughout employment
certified PRI assessor preferred
