Overview
The Care Manager plans and coordinates care management for members. The Care Manager works in collaboration with Care Manager Social Worker and Clinical Services Associate to collaborate on the plan of care for the member, and also monitor effectiveness of the care plan and promote member satisfaction with the service provision.
Responsibilities
Communicates with enrollment personnel to ensure a smooth transition at the start of care.
Educates members and family in health management and community resources.
Develops treatment plan, evaluates member progress regularly, and modifies treatment plans when necessary.
Provides direct services, such as counseling, psychotherapy, and case/care management.
Maintains oversight for the members care management during long-term SNF placements and short-term sub-acute placement for rehabilitation.
Qualifications
Education Requirements
Graduate of an accredited Nursing or Social Work Program
Education Desired
Bachelor's degree in Nursing or Social Work
Experience Requirements
Two (2) years of experience in a community based program caring for older adults.
Demonstrated skills in planning, organizing and directing the work of others required.
License Requirements
Registered Nurse, currently licensed and registered in New York State.
