• Assess the patients’ needs and identifies any barriers
• Carries out social evaluations and plans intervention based on evaluation findings.
• Maintains clinical records on all patients referred to social work.
• Connects patients and families with community resources, as needed

• Document your visits with our user-friendly technology, allowing for interdisciplinary communication and easy documentation

• Licensed Social Worker with current license in the state of Michigan
• Master of Social Work required
• Minimum of one year's experience in health care, home care experience preferred

• Valid driver's license and auto insurance
