Description

Position Title: Regional Nurse Consultant/PCC Expert Nurse

Department: Clinical

Reports To: Chief Clinical Officer

Position Summary: This role is responsible for clinical support to the facilities and will work Monday – Friday.

This role will be responsible for supporting the clinical teams at the facility

level and maintaining the electronic health record platform—Point Click Care.

Required Responsibilities/Accountabilities:

- Clinical support to facilities

o Resident record reviews

o AHC Clinical Processes education

o Clinical and PCC audits

o Assist with PNCs and POCs

o Maintain Nurse Resource Binders

o Assist with Skills Fairs

- Oversight of Return to Hospital prevention and management

- Assist with oversight of CNA School Program

- Oversight of Dript IV Therapy Compliance

- Collaborates with CCO for oversight of Carease partnership

- Complete routine facility rounding utilizing designated form

- Assist with survey management as needed

- User management of various clinical software including PCC, iCare, WoundRounds

- Assist with new vendor onboarding

- Other duties as assigned including direct resident care when necessary

Specific Educational/Vocational Requirements:

- Licensed Registered Nurse

Job/Technical Skills:

- Maintain work/availability Monday through Friday, although additional or off hours may be required on occasion

- This is not a remote position

- Excellent Customer Service Skills

- Friendly and effective communication skills—written and oral should be clear and concise

- Excellent leadership background paired with a strong focus in Long Term Care

- Strong organizational skills

- Basic computer skills including PCC, Word, Excel
